Instructions for Membership To PRAIRIE VOLLEYBALL CLUB WHAT DO I NEED TO DO TODAY? - Decide if your daughter will be part of the volleyball club. If you think your daughter is committed to playing for Prairie Volleyball Club (PVBC) then please make sure and fill out the Player Information Form and return it today.
- Fill out the Player Information Form and either turn it in today or mail it to :
Prairie Volleyball Club P.O. Box 452 Ely, IA 52227 WHAT DO I NEED TO DO AFTER THE PARENT MEETING? - If your daughter is in 6th, 7th or 8th grade she needs to attend the special placement practices. Times, dates and locations can be found under theImportant Updates tab on the volleyball website: www.prairievolleyballclub.org
- After November 1st but before November 15th sign up or renew your Iowa Region membership. Make sure that your daughter has been assigned a team before you do this. You will not get a refund from the Region.
- Go online to www.iavbreg.org and either sign up for membership or renew your previous membership for the current season.
- Follow the directions on the website. Parents…..make sure and have your driver’s license ready. You will need this to complete your membership.
- Print out the following forms. For many of these you have to open up the window read the form and close it. You will be allowed to print the form after doing so.
- Waiver and Release of Liability
- Participant Code of Conduct
- Code of Ethics
- Proof of current USAVB Membership
- Fill out and sign the Prairie Club Forms you were given at the time of the Parent Meeting.
- Player Code of Conduct
- Parent Code of Conduct
- Medical Release Forms – 2 copies
- It is very important to note that your daughter will not be allowed to practice until all of this information is turned in.
- Attend the Kick-Off meeting to be scheduled in November and bring with you all of the above forms, along with $50 deposit to be credited towards your daughter’s dues. Exact date, time and location can be found on the website: www.prairievolleyballclub.org
