2. If you are sure your daughter is going to play for the PVBC, then fill out the PVBC PLAYER INFORMATION FORM 

WHAT DO I NEED TO DO AFTER THE PARENT MEETING?:  (AFTER NOV. 1ST but BEFORE NOV. 15TH KICK-OFF MEETING)

<sup>ONCE YOUR DAUGHTER HAS A TEAM AND A COACH....
1. GO ONLINE TO www.iavbreg.org and SIGN UP OR RENEW YOUR REGIONAL MEMBERSHIP for this season. THIS MUST BE DONE PRIOR THE NOV. 15TH KICK OFF MEETING</sup>

2. Follow the directions on the Regional website, then FILL OUT and PRINT ALL FORMS indicated for your membership on the bottom of the application form.

3. PRINT AND BRING WITH YOU TO THE NOV. 15TH KICK OFF meeting COPIES of the following:

- Player Medical History & Release (we will need 2 copies - 1 for Club records & 1 for the Coaches to keep with them)

- Waiver & Release of Liability with Code of Conduct form (PRINT ALL PAGES)

- USA Online Confirmation that membership is active  (must show this form at the K/O meeting, and then keep in your daughters VB bag as proof)

- Age Waiver (if needed)  The website will prompt you if your daughter's age requires this form.

***COPIES OF ALL THESE FORMS MUST BE BROUGHT WITH YOU TO THE NOV. 15TH KICK OFF MEETING OR YOUR DAUGHTER CANNOT PRACTICE.

4. THEN FILL OUT & SIGN the following Prairie Volleyball Club forms you received today: (These forms are accessible below)

- Parent Code of Ethics (Must be signed by both parents)

- Player Code of Ethics Signature Page (Must be signed by parents & player)

-IF you didn’t turn in a PVBC Player Information Form at the Parent Meeting, THEN you must turn it in at Kick Off

***You NEED to bring all of these completed forms along with a check for $100.00 of your dues to the Nov. 15th Kick Off Meeting.
